SMS for dental practices: reminders that actually cut no-shows

No-shows cost a typical dental practice thousands of dollars a month, and the fix is boring: a reminder text two days before and a confirmation the morning of. Practices that text see reminder open rates near 98% versus roughly 20% for email. Here is how to run it without creating a compliance problem.

Get consent at booking, not later

Add one line to your intake or online booking form: "I agree to receive appointment reminders and practice updates by text at the number provided. Message and data rates may apply. Reply STOP to opt out." Keep the form, the date and the number. That single record is what carriers and the TCPA care about.

Choose the right number

  • Local 10DLC number with your area code feels familiar to patients. It needs a brand and campaign registration (a few days).
  • Toll-free number skips brand registration and only needs verification (one to three weeks). Good if you want to start without an EIN filing.

Three messages that do the work

  1. 48 hours before: "Smile Dental: reminder for your cleaning on Thu Sep 24 at 10:00am with Dr. Reyes. Reply C to confirm or call (405) 555-0100 to reschedule. Reply STOP to opt out."
  2. Morning of: "Smile Dental: see you today at 10:00am. 123 Main St, suite 4. Reply STOP to opt out."
  3. After the visit: "Thanks for visiting Smile Dental! If you have a minute, we would love a review: smile.example/review. Reply STOP to opt out."

Handle replies

Patients reply. "Running 10 late", "Can I move to Friday?", "Who is this?" A shared inbox means the front desk sees every reply in one place and answers from the practice number, not a personal phone. Replies to a patient are customer care, so they can go out any time.

Respect quiet hours

Marketing texts (recall campaigns, whitening promos) must stay between 8am and 9pm local time. Texttive holds anything marked marketing until the window opens; reminders and confirmations are transactional and send immediately.

Measure it

Track no-show rate before and after, and watch delivery status. A cluster of "undelivered" usually means landlines or bad numbers; run those through carrier lookup and clean them.
